You're excited about this opportunity because...
- You will collaborate with hospice clinicians to develop patient-centered plans of care.
- You will consult on the management of pain and other end-of-life symptom
- You will evaluate drug therapies for appropriateness, safety, effectiveness, and cost efficiency.
- You will advise clients on CMS Conditions of Participation and other regulatory reQuirements.
- You will provide timely responses to drug information inquiries and prepare educational material.
- You will participate in quality improvement initiatives and quarterly business reviews.
- You will contribute to developing technology resources and data tools that support client success.
We're excited about you because...
- You have an active, clear, and unrestricted Pharmacist license within your state of residence.
- You are a Registered Pharmacist in good standing with the Board of Pharmacy.
- You have a Bachelor's degree in Pharmacy or a PharmD degree.
- You have at least 1year of experience working as a licensed pharmacist.
- You are proficient in Microsoft Office and familiar with electronic medical record system
- You demonstrate excellent communication, clinical judgment, and problem-solving skills.
- You bring experience or training in hospice, palliative care, pain management or other pertinent clinical skills.
- You have completed a pharmacy residency or hold relevant certifications (BCGP, BCPS, CDP, etc.).
- You are passionate about serving patients and providers during end-of-life care with empathy and excellence.
The salary range for this position is $135,000-$145,000 plus a discretionary annual bonus.
