Care Pathway Manager

About Paloma Health

We aim to provide families with high-quality NHS children’s autism and ADHD assessments within 4 weeks of GP referral.

We were founded as we did not think it is right that families are waiting 2-5 years for NHS children’s autism and ADHD assessments, and this crisis has recently been highlighted by the Children’s Commissioner for England: https://www.childrenscommissioner.gov.uk/blog/childrens-commissioner-warns-of-invisible-crisis-as-delays-and-inequalities-laid-bare-among-children-with-neurodevelopmental-conditions/

Our success is driven by our fantastic team, as well as our constant focus on redesigning care pathways, building fantastic software, and innovating in care workforce models.

How is Paloma Health different?

  • We are clinically-led and Care Quality Commission (CQC) registered, with a friendly and approachable team
  • We have built our own software platform to:
    • Give our patients a more consumer-like experience of healthcare
    • Free our clinicians from admin to focus on patient care
  • Our strong culture, including transparency, a sense of community, and supporting staff career development
  • The experience of our founders (Dr Mark Jenkins & Darshak Shah), who have previously built tech-enabled NHS obesity and ophthalmology services. Combined, their previous companies have treated over 1 million NHS patients in the last 15 years!
  • Being backed by leading healthcare investors (Triple Point Ventures and Heal Capital), allows us to move fast

The Opportunity

Join our dynamic and collaborative multi-disciplinary team (comprising product, engineering, clinical, and delivery expertise) focused on transforming our neurodevelopmental services for children and young people!

As the Care Pathway Manager, you will drive continuous improvement in our autism and ADHD pathways for children and young people. You will work on improving how our delivery team delivers care to families, how families experience our services, and how our software platform supports both our delivery team and families.

This is an exciting opportunity for a highly organised and proactive individual with a passion for healthcare innovation to make a tangible difference in the lives of the families we support. You will be empowered to contribute to the strategic direction of our services, lead key elements of new projects, and foster a culture of continuous improvement.

This is not a traditional project management role where you solely report on progress against a static plan; you won't be confined to rigid processes or obsess over isolated features of an app. At Paloma, we are focused on agile delivery and improving the whole journey for the families we support. Your success is not measured in meticulous project plans but in how you impact service efficiency and family experience. Think of it less as standard project management and more as shaping a live service where you adapt, learn, and adjust continuously.

As you grow in the role, you may take on further responsibilities. Furthermore, with strong performance in the role, this person can be promoted to a Senior Pathway Manager.

Responsibilities

Collaboration & stakeholder engagement

  • Work as part of a small squad and collaborate closely with our clinical, operational and engineering teams.
  • Be a proactive and hands-on team member, willing to "jump in" and contribute directly to tasks and initiatives as needed.

Service optimisation & innovation

  • Be the expert on our neurodevelopmental services, identifying key areas for improvement, optimisation, and efficiency gains.
  • Champion an innovative and optimisation-focused mindset within the team, constantly seeking opportunities to enhance service quality and efficiency.

Project & Operational Management

  • Provide active support in the roll-out of new projects and workstreams, taking ownership of designated elements and ensuring successful implementation.
  • Regularly review key performance indicators (KPIs) and service data to identify trends, highlight areas requiring focus, and proactively suggest data-driven ideas and improvements to service delivery.
  • Manage the day-to-day operational aspects of the service, including updating relevant documentation, providing project management support, and organising and leading workshops and meetings.

What qualities will you bring to the role?

  • A strong drive and passion for improving healthcare outcomes and the experience of individuals accessing neurodevelopmental services.
  • A highly organised and structured approach to work, with exceptional attention to detail and the ability to manage multiple complex projects simultaneously.
  • A proactive and solution-oriented mindset, with a natural inclination to identify opportunities for improvement and drive positive change.
  • Excellent communication and interpersonal skills, with the ability to build strong working relationships across diverse teams.
  • A demonstrable ability to analyse data and identify key trends and insights to inform decision-making.
  • A flexible and adaptable approach, with a willingness to embrace new challenges and contribute to a fast-paced environment.
  • A proactive and "can-do" attitude with the ability to work effectively within a multi-disciplinary team.

Qualifications and experience

Required:

  • At least 3 years experience in a service improvement or similar role within a healthcare or similar complex service environment.
  • Proven ability to manage multiple projects concurrently and deliver excellent results to agreed timelines.
  • Demonstrable experience in analysing data and generating insights to drive improvements.
  • Previous experience in owning end-to-end projects with successful outcomes.
  • Strong organisational and administrative skills.
  • Excellent written and verbal communication skills.

Desirable:

  • Experience working within neurodevelopmental services or the wider healthcare sector.
  • Familiarity with continuous improvement methodologies (e.g., Lean, Six Sigma).
  • Qualification or training in project management.
  • Knowledge of relevant healthcare regulations and standards, e.g. the Care Quality Commission
  • Experience with digital healthcare platforms or technologies.
