Job Summary

We are seeking an experienced and highly organised Case Manager to oversee and coordinate a complex care package in Dublin. The role will ensure the delivery of safe, effective, and person-centred support, while managing the day-to-day operation of the package in line with best practice and regulatory standards

Key Responsibilities

  • Provide overall leadership, coordination, and oversight of the care package to ensure consistent, high-quality service delivery
  • Develop, implement, and regularly review weekly support plans and staffing schedules, responding to changing needs
  • Liaise with relevant stakeholders, including healthcare professionals, support staff, family members, and external agencies
  • Oversee medication management processes, including ordering, collection, storage, and administration protocols in line with HIQA standards and organisational policies
  • Coordinate shopping, household management, and daily living supports in accordance with the individual’s assessed needs and preferences
  • Act as the primary point of contact for family, ensuring appropriate access, communication, and involvement
  • Supervise, support, and guide care staff, including induction, ongoing supervision, and performance management
  • Ensure compliance with HIQA standards, safeguarding policies, and all relevant legislation
  • Monitor and manage allocated budgets and resources effectively
  • Maintain accurate, up-to-date records and reporting in line with regulatory and organisational requirements

 

Requirements

  • Proven experience in complex care, disability services, or case management
  • Strong organisational, leadership, and interpersonal skill
  • Experience overseeing care delivery, medication processes, and compliance framework
  • Ability to work independently while coordinating a multidisciplinary team
  • Full driving licence and access to a vehicle (desirable depending on package location
